• Maybe the man was a starving ghoul, or in the early stages of a zombie infection.
• Another explanation could be clinical lycanthropy - a mental disorder in which the sufferer believes himself to be a werewolf. In a suitable campaign this could result from encountering an actual werewolf and there will probably be equivalent disorders where people think they are zombies or what have you.
